眼针对大鼠脑缺血再灌注损伤不同时间点海马组织TNF-α表达的影响关洪全高原王素娟王哲马贤德赵金茹王守岩柴继严王健(辽宁中医药大学,辽宁沈阳110847)〔摘要〕目的观察眼针对大鼠脑缺血再灌注后肿瘤坏死因子-α(TNF-α)蛋白表达的影响,以探讨眼针对缺血再灌注大鼠脑海马区细胞的保护作用及其作用机制。方法健康SD 大鼠,雌雄不拘,设正常对照组、假手术组、模型组、眼针组;采用改良的线栓法复制大鼠大脑中动脉缺血再灌注损伤模型,选取眼穴肝区、肾区、上焦区、下焦区,平刺,进针3mm ,留针20min ;分别于脑缺血再灌注后3、24、72h 处死动物并取材;应用蛋白质印迹法(Western 印迹)检测海马组织中TNF-α蛋白的表达。结果眼针组海马组织TNF-α在脑缺血再灌注后3、24、72h 均较模型组明显降低,差异显著。结论眼针疗法可以降低脑缺血再灌注模型大鼠海马组织TNF-α的表达,进而抑制由脑缺血再灌注诱导的炎症反应,发挥脑保护作用。〔关键词〕眼针;缺血再灌注;肿瘤坏死因子-α;海马〔中图分类号〕R74〔文献标识码〕A〔文章编号〕1005-9202(2012)17-3705-03;doi :10.3969/j.issn.1005-9202.2012.17.043The effect of eye acupuncture therapy on the expression of TNF-αof hippocampus at different time points in rats with cerebral is-chemia reperfusion GUAN Hong-Quan ,GAO Yuan ,WANG Su-Juan ,et al .Liaoning University of Traditional Chinese Medicine ,Shenyang 110847,Liaoning ,China【Abstract 】Objective To investigate TNF-αexpression at different time points in the brain of rats with cerebral ischemia-reperfu-sion (IR )injury ,in order to discuss the mechanism of eye-acupuncture therapy and observe the intervention effects on the hippocampus.Methods Healthy SD rats ,male and female ,were randomly divided into normal ,sham-operated control ,model and eye-acupuncturegroups.Sham-operated control ,model and eye-acupuncture groups were divided into 3,24and 72h subgroups.The rat model of cerebral IRwas established by suture method.Eye acupoint liver area ,kidney area ,upper jiao area and lower jiao area were selected and acupunctured horizontally ,inserting needle in 3mm deep ,retaining needle for 20min.All the animals were executed respectively at 3,24,72h after reperfusion and draw materials.The method of immunohistochemistry and Western blot technology were taken to detect the changes of hippo-campus TNF-αexpression in rats after the eye-acupuncture therapy.Results Compared with that of model group ,TNF-αexpression in thehippocampus of rats of the eye-acupuncture group was significantly decreased obviously at 3,24,72h after reperfusion.Conclusions Theeye-acupuncture therapy could down-regulate hippocampus TNF-αexpression in rats with cerebral IR injury ,suppress inflammatory responseinduced by cerebral IR and play a role in protecting brain.【Key words 】Eye acupuncture ;Ischemia-reperfusion ;Tumor necrosis factor-α;Hippocampus基金项目:国家重点基础研究发展计划项目(No.2007CB512702)通讯作者:王健(1948-),男,主任医师,主要从事中西医结合治疗缺血性脑血管病研究。第一作者:关洪全(1954-),男,教授,主要从事中医药对免疫的调控研究。眼针疗法是辽宁中医药大学附属医院彭静山老先生发明的一种针刺疗法,临床应用多年,对缺血性脑卒中等疾病具有很好的疗效,但其机制尚未阐明。脑缺血再灌注损伤主要与氧化应激反应、炎性反应、钙超载、脑水肿和细胞凋亡等有关。炎性反应在脑缺血再灌注损伤中起着重要作用。肿瘤坏死因子-α(TNF-α)是一种具有广泛生物学功能的炎症介质,在脑组织缺血再灌注后引起的炎症反应、细胞凋亡等组织损伤中发挥重要作用〔1 3〕。本研究通过观察眼针对大鼠脑缺血再灌注损伤后脑海马组织匀浆中TNF-α蛋白表达的影响,探讨其脑保护作用机制。1材料与方法1.1实验动物与分组健康SD 大鼠96只,雌雄不拘,体重(280ʃ20)g ,由北京维通利华实验动物中心提供。实验前适应性喂养1w ,自由饮水、摄取标准颗粒饲料,室内温度22ħ,相对湿度45%。将大鼠随机分为正常对照组、假手术组、模型组、眼针组,再根据缺血再灌注损伤时间分设3h 、24h 、72h 亚组,每亚组8只。1.2模型制备与评价模型组及眼针组采用改良的线栓法复制大鼠大脑中动脉缺血再灌注损伤模型,模型制作按照文献〔4〕记载方法进行。模型成功标准:大鼠于缺血2h 后进行再灌注,再灌注后进行神经功能缺损评分,参照Longa 5分制评分标准〔5〕:0分为无症状;1分为不能完全伸展对侧前爪;2分为向对侧转圈;3分为向对侧倾倒;4分为不能自发行走,意识丧失。评分1 3分者纳入实验组,未达标准者排除。假手术组术式同模型组、眼针组,区别在于钓鱼线插入深度为0.5 1cm ,其他同手术组。正常组未处理。1.3眼针取穴及刺法眼针组:用31号13mm 毫针,于大鼠眶周2mm 处针刺,定位参照人体取穴方法〔6〕,取肝区、上焦区、下焦区、肾区针刺。手法:平刺,进针3mm 。留针10min 时用刮柄进行刮针1次,刮针5下。治疗时间:3h 针刺组于脑缺血
