基于神经解剖学温针灸与穴位贴敷治疗脑卒中痉挛性偏瘫的临床研究苟娟平（陕西中医药大学基础医学院，陕西咸阳712046）［摘要］目的观察基于神经解剖学温针灸与穴位贴敷治疗脑卒中痉挛性偏瘫的临床疗效。方法将102例脑卒中痉挛性偏瘫患者随机分为2组，对照组51例应用穴位贴敷治疗，研究组51例在对照组治疗基础上应用基于神经解剖学温针灸治疗，2组均持续治疗4周。记录2组治疗前后Ashworth 痉挛量表评分、临床痉挛指数（CSI ）、Fugl －Meyer 运动功能评分以及Barthel 指数评分。结果2组治疗后Ashworth 痉挛量表评分和CSI 指数均较治疗前显著下降（P 均＜0．05），且研究组降低程度均显著优于对照组（P 均＜0.05）；2组治疗后Fugl －Meyer 运动功能评分和Barthel 指数评分均较治疗前显著上升（P 均＜0.05），且研究组升高程度均显著优于对照组（P 均＜0．05）。结论基于神经解剖学温针灸与穴位贴敷治疗脑卒中痉挛性偏瘫疗效显著。［关键词］神经解剖学；温针灸；穴位贴敷；脑卒中；痉挛性偏瘫doi ：10．3969/j．issn．1008－8849．2019．10．008［中图分类号］Ｒ743．3［文献标识码］A ［文章编号］1008－8849（2019）10－1061－04Clinical study on treatment of spasmodic hemiplegia with warm acupunctureand acupoint application based on neuroanatomyGou Juanping（Basic Medical College ，Shaanxi University of Traditional Chinese Medicine ，Xianyang 712046，Shaanxi ，China ）Abstract ：Objective It is to observe the clinical effect of warming acupuncture plus acupoint application on spastic hemiplegia based on neuroanatomy．Methods 102patients with hemiplegia due to stroke were randomly divided into 2groups．51patients in the control group were treated with acupoint application．51patients in the study group were treated with warm acupuncture based on neuroanatomy on the basis of the control group ，and both groups were treated continuously for 4weeks．The Ashworth sputum score ，clinical sputum index （CSI ），Fugl-Meyer motor function score ，and Barthel in-dex score were recorded before and after treatment in the two groups．Ｒesults After treatment ，the Ashworth sputum score and CSI index were significantly lower than those before treatment （P ＜0．05），and the Ashworth sputum scale and CSI in-dex of the study group were significantly lower than the control group （P ＜0．05）．The Fugl-Meyer motor function score and Barthel index score were significantly higher than those before treatment in the two groups （P ＜0．05），and the Fugl-Meyer motor function score and Barthel index score in the study group were significantly higher than the control group （P ＜0.05）．Conclusion Warm acupuncture based on neuroanatomy plus acupoint sticking has good curative effect on hemiple-gia due to stroke．Key words ：neuroanatomy ；warm acupuncture ；acupoint application ；stroke ；spastic hemiplegia ［作者简介］苟娟平，女，硕士，助理实验员，研究方向为针刺治疗脑病的机制与临床。脑卒中是临床常见脑血管疾病，具有发病率高、病死率高和复发率高等特点，易形成痉挛性偏瘫后遗症，造成肢体功能障碍，严重影响患者日常生活活动能力［1］。目前，现代医学多采用口服乙哌立松或巴氯芬、配合康复锻炼等方法治疗脑卒中痉挛性偏瘫，虽能部分缓解肌张力，但持续时间较短，且药物毒副作用大，治疗效果明显受限［2］。研究发现，中医温针灸疗法不仅能发挥温通经脉、行气活血止痛、补虚扶弱之功效，还能基于现代神经解剖学，通过针刺腧穴激发经气，改善局部微循环，调节机体脏腑功能，有效治疗脑血管疾病［3］。本研究观察了基于神·1601·现代中西医结合杂志Modern Journal of Integrated Traditional Chinese and Western Medicine 2019Apr ，28（10）
